 Requesting Copies of Vital Records

Copies of Birth, Death, Marriage, Divorce and Domestic Partnership Certificates can be acquired at the Register of Deeds Office.  With the new statewide issuance law now in effect, if the vital record you are requesting falls within the dates listed below, you can obtain the copies from ANY Register of Deeds in the State of Wisconsin.

               Birth Certificates - from 10/1/1907 to present               

                     Death Certificates - from 9/1/2013 to present                    

                       Marriage Certificates - 1907 to present                      

                      Divorce Certificates - 1/1/2016 to present                     

                  Tribal Marriage or Divorce - 1/1/2016 to present                 


 There are 3 ways to submit a request for vital records, along with guidelines that must be followed for each.

  • Applying in person/la solicitud en persona
    • Identification must be presented with all applications (click here for acceptable forms of ID)
    • La identificación debe ser presentada con todas las aplicaciones (haga clic aquí para las formas aceptables de identificación )
    • Certified copies require a direct and tangible interest as defined on the application
    • Applications are available at the ROD service desk or online below
    • Please arrive a minimum of 15 minutes prior to closing time to allow time for processing
  • Application by US Mail
    • Identification must be provided with all applications (click here for acceptable forms of ID)
    • Online application forms are available below
    • All requests made by mail are processed the day they are received. All submissions must include a completed application, a photocopy of current acceptable identification, the correct fee, and a business size self-addressed stamped envelope.
  • Online Application (Primarily for out-of-state residence)
  • Outagamie County Register of Deeds uses the VitalChek Network, Inc. service to process all online vital records requests

The fee for all vital records is $20.00 for the first copy and $3.00 for each additional copy.
