Invest in the Future: Become a Mentor
Thank you for your interest in The Outagamie County Mentoring Program. When you are ready to become a mentor, print and complete our Application, Background Check, and Records Check forms and mail to:
Outagamie County Mentoring Program
Youth & Family Services Division
500 W. Fifth Street
Appleton, WI 54911-6008
We are committed to making successful matches for the youth referred to our program, as well as for our mentors. With this in mind, our initial screening and interview process of potential mentors does take some time. We believe the time spent in the beginning leads to successful matches in the end. Once we receive your application packet, we will begin to process the background checks and obtain your personal references for our review. As soon as these are complete, we will contact you to set up an in-home interview. Your mentor application packet will then be reviewed by staff and the program supervisor for approval. Once approved, we can start to look at our waiting list of youth!
Please contact us if you have questions about our mentoring program, the application process, or the requirements and commitment required to be a mentor.
