Why do we have a Community Health Improvement Plan?
Since 1993, Wisconsin statutes (Ch. 251.05, DHS 140.04) have required communities throughout Wisconsin to develop and implement local health plans to address health conditions impacting their residents. This process has been referred to as the Community Health Improvement Process (CHIP). The CHIP consists of a Community Health Assessment and a Community Health Improvement Plan.
