The Aging and Disability Resource Center (ADRC) is here to help! Transitioning from school to adulthood should be an exciting time, but can often be overwhelming and uncertain especially for those who have a disability. The Aging and Disability Resource Center (ADRC) of Outagamie County is here to help you and your loved one with the transition. The ADRC can help you navigate your options, find resources in the community that will work best for you, and provide information about eligibility for appropriate benefits and explain how to apply.

Help for Older People and Adults with Disabilities

Tell me more about enrolling in Publicly Funded Long-Term Care Programs

The ADRC handles the functional eligibility and assists with financial eligibility determinations for Long Term Care funding. The student and family can request this assessment at any time, as long as the student is 17.5 years of age. Long Term Care funding can assist a person with supports in the community, in the home, and has options for alternative living. Guardianship Support Center

The ADRC has resources available to assist you with the process of assuring you have a decision maker for individuals that are not able to navigate decisions on their own. The Guardianship Support Center https://gwaar.org/guardianship-resources is a great place to start to learn about the different options of a decision maker for a youth that will turn 18.
