The Aging and Disability Resource Center (ADRC) is here to help!  Transitioning from school to adulthood should be an exciting time, but can often be overwhelming and uncertain especially for those who have a disability.  The Aging and Disability Resource Center (ADRC) of Outagamie County is here to help you and your loved one with the transition.  The ADRC can help you navigate your options, find resources in the community that will work best for you, and provide information about eligibility for appropriate benefits and explain how to apply.

If you need assistance or are just not sure what to expect, call the Aging and Disability Resource Center, 920-832-5178 or email our general mailbox at adrc@outagamie.org to get your questions answered and start the process of transition to adult services or a decision maker for your loved one.
